Division and Program Description
The Harbor Haven Supportive Housing Program is an innovative supportive housing program based in San Francisco's Bayview/Hunters Point community. Funded through the U.S. Department for Housing and Urban Development (HUD); Harbor Haven is a community-based housing program. The goal of Harbor Haven Supportive Housing Program is to integrate permanent supportive housing with support services that will assist chronically homeless and disabled individuals with meeting their basic needs, establishing a sense of self mastery, and living a life of housing stability. Program staff work directly with participants to access and maintain support services by working on three fronts: 1) Providing permanent supportive housing for the chronically homeless in a community living environment; 2) Working with various agencies to address and support participants' needs in regards to mental/physical health and/or substance abuse; 3) Work with multiple agencies to assist program participants in accessing the employment/volunteer training programs to increase likelihood of economic improvement. Case Managers serve as liaisons and advocates that provide: supportive administrative coordination between program participants, collaborating agencies and other accessible resources.
Job Description
- Maintaining the facility: Ensuring the facility runs smoothly, and that clients receive appropriate care.
- Supporting residents: Helping residents maintain their tenancies and improve their quality of life.
- Planning events: Coordinating special events and activities
- Providing crisis intervention: Overseeing and providing crisis intervention, case management, and follow-up services.
- Maintaining records: Keeping records of services and billing, and submitting reports as required.
- Developing treatment plans: Creating and monitoring treatment plans and progress notes.
- Referring clients to community resources: Facilitating referrals and linkages for employment, healthcare, legal, educational, and peer-support
- Maintaining communication: Maintaining consistent communication with clients to assess their wellbeing and make changes to their treatment plan as needed.
Minimum Requirements and Qualifications
- High School diploma or
- Previous related experience.
- Spoken and written English adequate to meet the needs of the job.
- Must be able to lift 25-50 pounds.

FLSA Status: Exempt
Salary Range: $60,000 - $75,000 Annually (DOE)
